About Rachel

I am a registered social worker with five years practice experience. Prior to qualifying in Social Work, I worked with young people and adult's with complex needs and disabilities for sixteen years.

As a Registered Social Worker within the Learning Disability and Autism sector, this experience has offered opportunities to practice with a diverse range of needs including offending, drug and alcohol, mental health conditions, forensic social work, disabilities, Domestic Violence, Trauma-Informed practice, Child and Adult safeguarding, Child Sexual Exploitation, working with individuals detained in hospital under the Mental Health Act 1983, providing reports for Mental Health Review Tribunals and reintroducing individuals back into the community.

More recently working with women allocated to my team trafficked to the United Kingdom having experienced Modern Slavery and violence.

I coordinate a wide range of complex care and support. Additionally, as part of my role I have a high level of responsibility for care and support teams providing specialist support to individuals with managed care needs. The range of experience supports my ability to respond and understand the needs of the individual and the factors surrounding their circumstances, enabling a more informed, precise and wider response to support.

I have practiced in this area for twenty years. My role as a Care Coordinator in the team encompasses case management and care management, involving direct clinical and social care interventions delivered to individuals and coordinating the Multi-Disciplinary Team around the individual within the delivery system to ensure that care is appropriate, timely, and safe. Additionally, addressing the needs of individuals: from a multi-dimensional lens of biophysical, psychological, sociological, and spiritual assessment. The focus is on providing appropriate and consistent access to care, while empowering individuals as they navigate transitions.

I have experience of undertaking specialist needs assessment, communication assessments, risk assessments, risk intervention plans, functional analysis behaviour plans, and managing complex cases, in addition to planning appropriate and targeted interventions to reduce the risk of high risk issues and behaviours, offending/re-offending and much more. Additionally having experience to identify cognitive challenges and the impact on the social, emotional and environmental factors on an individual.

About the lesson

I base my teaching and mentoring of putting theory to practice. I offer a bespoke teaching opportunity to individuals with a wide variety of needs and experience. As part of my mentoring, I also provide students with emotional and practical advice.
